Note:  Pfizer-BioNTech and Moderna COVID-19 vaccines use different age groups for their children’s vaccines.  For more information click on the vaccine name under the category that best refers to you. 

Adults ages 18 years and older:

PLEASE BRING YOUR COVID-19 VACCINATION CARD

You cannot receive the COVID-19 vaccine if:
• You are currently isolated and have not recovered from COVID-19
• You have pending test results due to an exposure
• You have received convalescent therapy for COVID-19 in the last 90 days
